Medical Membership Quick Fact Sheet
What Is It?
- It is a monthly membership program for individuals and families to receive services at Alaska Urgent Care and Wasilla Medical Clinic in exchange for a monthly fee.
- Our goal is to provide affordable and accessible healthcare for individuals and families who cannot afford health insurance.
Who Qualifies?
- Anyone who is uninsured and DOES NOT qualify for Medicaid or Medicare (per Alaska laws).
Who Can Be On A Membership?
- Anyone can be included – Family members, significant others, extended family, etc.
What is included:
- Plan 1: 5 visits per year
- Plan 2: 12 visits per year
- Can be used at either location
- Can be used by anyone in the membership
- Can be used for annual physicals, wellness checks, illness, and/or injuries, etc.
- A sports, school, or camp physical for any children on a membership.
- TB test (PPD) for all adults on the membership.
- Point of Care Tests (strep, flu, urinalysis, etc.)
- Other lab tests (send-out labs such as blood work and cultures)
- X-rays relevant to medical visits.
What is the Cost?
- $50/month for Plan 1 (includes all items listed above)
- $100/month for Plan 2 (includes all items listed above)
- Employers can cover all or part of the cost for employees. – Reach out to the contact below to set up employer payments.
